 As a Care Manager you will need to have Knowledge in all aspects of the electronic medical record, including training and monitoring for compliance and timely follow-up. You will be responsible for the daily nurse visit schedule that includes billing and charting in a timely manner. Assist with flu shot and other immunization clinics in the capacity of an RN, duties include Triage, facilitating patient questions and answers and case management: Per Medical Home, HEDIS and Meaningful Use requirements.  Respond to actual or potential health problems through case finding and provision of care supportive to restorative life and well-being. To execute medical regimens as prescribed by a licensed provider, including but not limited to, tests, treatments and medications. To participate in quality improvement, in-service workshops, continuing education and peer review as is requested by management and supervision. To engage in Patient Centered Medical Home initiatives, including participating as part of the Clinical Care Team. Any other duties and responsibilities as requested by management and supervision.

Job Requirements: Graduate of an accredited RN nursing program, with a current New York State license as a Registered Nurse. Demonstrate a willingness to accept direction from providers and VP/ Chief Nursing Officer.  Must possess the ability to work well and with compassion in stressful situations, maintain a professional and courteous demeanor with both patients and co-workers and be keenly aware of the importance of confidentiality in all aspects of the position.

Maintain a timely flow of patients according to the day’s schedule. To perform task and responsibilities within the framework of case finding, health teaching, health counseling, and provision of supportive and restorative care under the direction of a registered professional nurse or licensed physician, dentist or other licensed care provider legally authorized under this title and in accordance with the commissioner’s regulation. Must also have a strong focus on Prescription Preparation Process. Complete other duties and responsibilities as requested by management and supervision.

Job Requirements: Graduate of a certified Nursing Program, with a license to practice practical nursing. Pediatric Experience preferred for School Based position. To work with the behavioral health providers, medical staff, nursing staff, management team to assure clinical quality of the highest standard. This position counsels patients with emotional, developmental and/or personality problems, referring severe cases to appropriate community or medical agencies. Must have the ability to carry full patient load, including all necessary documentation. Complete other duties and responsibilities as requested by management and supervision. Supervision provided. Experience with electronic health records (EHR) and electronic medical records (EMR) strongly preferred.

Job Requirements: Graduate of an accredited health institution with certification in NYS, Masters Degree with clinical licensure. Patient Access Representative

As a Patient Access Representative with NOCHSI you must foster patient service skills acting as a patient advocate, making certain all of their needs are met This position will greet patients, visitors and representatives to the center, directing them to appropriate offices. Additional tasks are; registering all patients entering the office, verifying demographic and insurance coverage (Medicaid, Medicaid Managed Care) and initiating encounters for patients seen. This position also answers all incoming calls scheduling appointments as appropriate and scanning documents into EHR as received. Must comply with organizations code of conduct, safety rules and adheres to all company policies.

Job Requirements: High School Graduate/ Equivalency, with medical/office billing experience preferred. Must be courteous and tactful. Must possess the ability to firmly purse payment on accounts. Must be keenly aware of the importance of confidentiality in all aspect of the position.

Orders Management Assistant

Review and follow-up on all open laboratory and diagnostic services procedures ( i.e. radiology, CT and MRI referral orders). Communicate with patients to remind them of services via telephone or patient portal. Mail reminder orders for tests due if not completed within 30 days. Monitor images for laboratory and diagnostic services for test results and close the open order when results are received. Communicate with Outpatient Radiology centers if results have not been received to obtain reports. Answer phone calls and pages, coordinating communication with team members and patients. Insure to communicate any report labeled with critical results to provider through a warm handoff. Engage in Patient Centered Medical Home initiatives, including participating as part of the Care Team through huddles. Any other duties and responsibilities as requested by management and supervisor.

Dental Assistant

As a Dental Assistant, you will act as chair side assistant to the dentist, preparing necessary materials and equipment for various dental procedures. Cleaning, sterilizing and setting up instruments, preparing exam rooms and setup between patients, taking x-rays, and performing routine maintenance on dental equipment. Assisting with maintaining dental patient record, completing the encounter form and ensuring all services rendered are documented. Dental Assistants work closely with the dentist and dental staff in improving productivity and efficiency and participate with in-service workshops and quality assurance activities as required. This position will include scheduled hours at both our Pulaski and Fulton Dental offices.

Facilities Staff

As a Facilities Staff member with NOCHSI you will maintain and clean all areas of the facility including patient rooms, restrooms, hallways and the lobby area. Must be able to vacuum and mop floors, some lifting is required. Small maintenance duties might be necessary. Must be flexible with schedule and able to travel, if necessary, to other sites and be willing to take direction from the Facilities Manager.
